“I hate these bags under my eyes.” It’s one of the most common complaints we hear, because those dreaded eyelid bags really can make us look tired, sad, or just plain old. Plus, they can appear at any age. So, what’s the best option to treat them?

You have three basic option for eyelid rejuvenation treatment: injectables (Botox or Fillers), fat grafting (fat injections to the face), or eyelid surgery. Our patients in this example chose a combination of all three.

Rejuvenating the eyelids with injectables

Injectables are the most popular cosmetic treatment by leaps and bounds, and can be excellent for restoring a more refreshed appearance to the eyes. Dermal fillers (like Juvéderm or Restylane) can help fill the hollow under the eyes to smooth out eyelid bags, while neurotoxins (Botox, Dysport or Xeomin) can further refresh your appearance by smoothing wrinkles in the upper face, such as crow’s feet or frown lines.

However, fillers are not for everyone—in some patients they can actually make eyelid bags worse. That’s why Dr. Ditesheim does all of his own injections at our office. As a board certified plastic surgeon, he has the skill and knowledge of facial anatomy required to ensure great results with injectables. Also, keep in mind that injectables are temporary, and your results will need touching up after approximately 9 months to stay looking their best.

Fat grafting as a permanent filler

Another option to treat eyelid bags without surgery is with facial fat grafting. This involves harvesting fat with liposuction from one area of the body and strategically re-injecting it to restore a smooth contour beneath the eyes. Unlike injectable products, fat can last permanently, eliminating the need for follow-up treatment (although nothing can stop the normal aging process). Our patient to the left had fat injections to the lower eyelids with Botox to her glabellar lines, and she loves her more youthful appearance.

Eyelid surgery for lasting, dramatic improvements

While non-surgical treatments can be a great option, many patients see the best results with eyelid lift surgery. During this procedure, Dr. Ditesheim removes excess skin and repositions the fat and muscles around the eyelids to restore a smoother, more open appearance. The incisions are hidden in the natural eyelid creases, so they’re barely noticeable. Our patient on the right had eyelid surgery after first trying injectable fillers—she liked her results with fillers, but only surgery gave her the results she was really looking for.
